From stress fracture to a marathon personal best

The challenge
A 41-year-old marathoner arrived with a tibial stress reaction after four failed comeback attempts. Each return to running collapsed within three weeks. Gait analysis revealed an overstriding pattern loading the tibia at 1.4× the expected force.
The approach
We rebuilt his running from zero: 12 weeks of progressive bone loading and calf-soleus capacity work, followed by cadence retraining in the gait lab and a graded return-to-running algorithm with weekly load monitoring.
The outcome
He returned to full training in five months and ran the Berlin Marathon seven months after intake — a personal best of 3:04, at age 41, with zero pain days in the final training block.
